Immunization Requirements

All new students are required to submit immunization information.  Please follow the link to the secure Student Health web portal at to record your immunizations.

In addition, the following form must be completed by your health care provider, signed and returned to Student Health Services before July 31st, for the fall semester, and January 20th, for the spring semester. Failure to complete the requirements by July 31, for the fall semester, and January 20, for the spring semester may result in a $100.00 immunization late fee, a delay in housing check in and possible delay in course registration.

Physical examinations are not required for admission to the University. (Athletics has their own requirements; please contact the Athletics Department for specific requirements at 410.543.6355.)

Required Immunizations:

Student Health Services requires new students to submit documentation of the following:

  • Meningitis (Meningococcal Vaccine)
    Maryland law requires that all students, graduate and undergraduate, enrolled at the University and who reside in campus housing have the Meningococcal vaccine or a signed waiver on file in SHS.
  • Measles, Mumps and Rubella (MMR)
    Two doses of the MMR, usually given in early childhood, should be evident on the immunization record.
  • Tetanus-Diphtheria-Pertussis (Tdap)
    A single dose of Adult Tdap (Adacel or Boostrix) given at > 11 years of age AND after 5/2005 (date of FDA licensure). Pediatric DTap given < 11 years of age or before 5/2005 will not be accepted
  • Tuberculosis Screening
    US citizens must complete a TB screening form. If TB risk factors are present, you must obtain a TB test within 6 months of your academic term start. Complete the online form on the secure Student Health web portal at

    All international students on visas must have a TB test regardless of BCG vaccine. If you have had a TB skin test, blood test or chest x-ray within 6 months of arrival to Salisbury University, please provide the documentation, including the x-ray results (in English). Please do not bring the X-ray film. Recommended Immunizations:

  • Hepatitis B vaccination is strongly recommended and is required for nursing and health majors.
  • Varicella Vaccine, required for nursing and health majors.
  •  HPV vaccine - Gardasil is strongly recommended (for both women and men)
