Request Forms

Assessment Development Request

Instructional Design & Delivery provides a faculty service for the development of online assessments. Faculty can provide ID&D with a Word file with assessment questions and associated correct answers and our staff will format the assessment and format the assessment in your MyClasses course.

To use this service faculty can send an email to suiddstaff@salisbury.edu with the following:

Please provide 2 working days for the processing of any requests. Requests received less than 2 working days will be completed as possible.

Clicker Adoption Form

Instructors must complete the Clicker Adoption Form to indicate that they will be using Clickers in your classroom for the upcoming semester. The form MUST be completed every semester to ensure the classroom is “clicker ready” and to ensure that students are aware of the required device/subscription as part of the required course materials.

Closed Caption & Transcript Request Form

Instructional Design & Delivery can help guide faculty on the best way to include captions or transcripts with their content based on the type of media file, software used, and hosting space.

For  help requesting these materials, please submit a request through ID&D’s Closed Caption & Transcript Request Form.

Course Review Request Form

Faculty can request for their online or hybrid course to be peer-reviewed using the QM rubric.  Salisbury University offers three types of reviews:

  • ID&D Course Review - An Instructional Designer will review the course based on the QM rubric and setup a consultation with the faculty or course developer.
  • Internal QM Review - Internal quality assurance review for courses/departments not interested in official QM reviews or a preliminary review prior to an official QM review.
  • Official QM Review - SU managed official QM review comprising of a three member team (subject matter expert, internal and external reviewer).

You can request any of the three review types using the Course Review Request Form.

Equipment Reservation Form

Instructional Design & Delivery has several items of equipment in which SU faculty can borrow for course and content development. The current existing equipment is:

  • Portable Digital Projectors
  • Clicker Sets (Student Response System)
  • Tablet Computers
  • Laptop Computers

To borrow one of these items, please complete the Equipment Reservation Form.

MyClasses Combine Sections Request

If you are teaching more than one section of a course in the same semester, you may choose to combine your sections in MyClasses. This allows you to develop and manage content in once course space and provide the content to both sections. In order to have these sections combined, please fill out the Combine Sections Request.

MyClasses Non-Credit Course or Group Site Request

Non-Credit bearing courses/sites can be created within MyClasses Canvas to assist faculty and staff with course development and program/project delivery/management. Examples include:

  • Development Course
  • Non-Credit or Organizational Course
  • Group Site where all individuals have equal access
