Immunization Requirements
All incoming Salisbury University students are required to submit immunization information. Please access the Student Health web portal to (1) record the dates of your immunizations, (2) upload a copy of your immunization record or Pre-Entrance Immunization Record Form and (3) complete the Tuberculosis Screening Questionnaire.
Incoming students must provide documentation of their immunization record in addition to date entry on the Student Health Portal by submitting one of the following: a copy of the official immunization record or the Pre-Entrance Immunization Record Form.
If you received your immunizations in Maryland, you may have access to your immunization record at the Maryland ImmuNet Website. Immunization records may also be accessed via childhood medical providers and/or public schools.
If you have a medical contraindication or religious exemption to vaccines, please complete the vaccination exemption form and upload it to the secure student health portal.
Failure to complete requirements by August 1st for the fall semester or January 15th for the spring semester will result in a $100 immunization late fee that is nonrefundable, a delay in housing move-in, and a hold being placed on your account for course registration.
Physical examinations are not required for general student admission to Salisbury University. Athletes have additional requirements to those from Student Health Services; please contact the Athletic Department for questions regarding these requirements.
Required Immunizations
Student Health Services requires new students to submit documentation of the following:
International Students
All international students on visas must have a TB test preferably the Quantiferon Gold regardless of BCG vaccine. If you have had a TB skin test, blood test or chest x-ray within 6 months of arrival to Salisbury University, please provide the documentation, including the x-ray results (in English). Please do not bring the X-ray film. If you have not had a TB test, please have a QuantiFERON Gold Test or T-Spot blood test done and submit documentation via email: studenthealth@salisbury.edu, fax:410-548-4101, or mail: Student Health Services, Salisbury University, HH 180, Salisbury, MD 21801. Or, you can also upload an image of your form directly into the Student Health Web Portal.
- Tuberculosis Test Documentation Form (PDF) - This form is ONLY NEEDED for all international students, and students who answer “YES” to any of the questions on the TB Screening Questionnaire form located online in the Secure web portal.

SHS offers in-house QuantiFERON Gold testing by appointment only on Mondays & Thursdays. The fee for this service is $25. Please call the front desk to make an appt (410-543-6262) or self schedule your appt online via the student health portal.
Recommended Immunizations:
- COVID-19: Salisbury University strongly encourages all members of the campus community to receive the COVID-19 vaccine and all recommended boosters.
- Hepatitis B vaccination is strongly recommended and is required for nursing and health majors.
- Varicella Vaccine, required for nursing and health majors.
- HPV vaccine - Gardasil is strongly recommended (for both women and men)
